Bassetlaw Community and Voluntary Service are looking to recruit Social Prescribing Link Workers who will be based within Bassetlaw’s Primary Care Networks (PCN), 37.5 hours per week. The Primary Care Networks are designed to link staff from general practice with other local services to deliver joined-up care for the local population.

As a Link Worker, you will be responsible for providing personalised support to individuals and their families, in order to empower them to take control of their own health and wellbeing, give them time and focus on ‘what matters to me’, in order to holistically improve an individual’s health and wellbeing.

In addition, you will be required to forge strong links with local voluntary and community sector organisations, providing a two-way link between them and Bassetlaw GP Practices and statutory services. A key part of this role will be to develop new support groups and strengthen community resilience. You will work in partnership to proactively raise awareness of social prescribing and its benefits. Ideally, you will have a strong knowledge of the Bassetlaw District.

The successful candidate will have the ability to motivate and influence others through a person-centred approach. You will be passionate, dedicated and empathetic and enjoy the challenge of working across new partnerships. Being able to travel around the district is a key requirement of this role.
